I am so sorry that your name was misspelled in the {marketing materials/newsletter/program/etc.}. I am truly mortified. Even worse, I expect that you rightfully wanted to keep the item as a memento.
I know it's too late in this case, but I am doing everything I can to make sure that misspellings don't occur in the future. I've instituted new proofreading measures and other steps to prevent such an error.
I've enclosed {gift card/in-house reprint/etc.}. And, again, I'm so very sorry about this.
