I was mortified when {supervisor/client/etc.} pointed out that copyright was violated by our company, and shocked to see that it happened on my watch. At first, I couldn't believe I had been involved in {decision}, but upon checking my records I've confirmed that the mistake was mine. I indeed reviewed and approved the {article/content} in question. I have no memory of why I would have thought it was OK to use this material without permission. I take copyright very seriously and will not let it happen again.
